Social Prescribing
A social prescriber is a link worker who will signpost patients to local social interventions and opportunities. There is lack of robust evidence to support it as an intervention in its own right.
Referral criteria
- Aged 18yr or older, with psychosocial needs (i.e. loneliness, social isolation, stress, anxiety, low mood, benefits, money or housing problems, requiring support with weight management or carer) impacting on their wellbeing.

Required information prior to referral
- Consent.
- A willingness to engage.
