If cases are to be considered for special consideration it needs to be demonstrated that the patient is significantly different to the general population of patients with the condition in question. In addition they must be likely to gain significantly more health benefits from the intervention than might normally be expected with patients with that condition.
For example, larger breasts will often cause back and shoulder pain, skin infections and emotional distress. Referrers will need to evidence significant difference over and above these expected symptoms.
Appropriate cases should be referred with as much information as possible including photographs, using the application form. The form and further information and guidance can be found here
