Non-diabetic hyperglycaemia referrals to National Diabetes Prevention Programme
Referral criteria:
WW (previously Weight Watchers) is the provider in Cornwall and the Isles of Scilly for the National Diabetes Prevention Programme (NDPP), known as ‘Healthier You’.
Healthier you is available free to people who meet the following eligibility criteria:
- HbA1c of 42-47mmol/mol (6.0 percent to 6.4 percent), or;
- Fasting Plasma Glucose (FPG) of 5.5 to 6.9mmol/l, or;
- Oral Glucose Tolerance Test (75g load) two hour result of 7.8-11.0mmol/l
- Is 18 or older (no upper limit).
- Is not pregnant
- Has non-diabetic hyperglycaemia (NDH) identified by blood test within the past 12 months.
- Does not have blood results suggesting Type 2 diabetes.
Anyone referred to the programme will be offered the opportunity to attend for a nine-month period of regular intervention and support.
Participants have full access to all the WW digital tools, including a curriculum proven to reduce the risk of developing Type 2 diabetes, one-to-one support, access to recipes, 24/7 online chat, and community groups dedicated to the prevention of Type 2 diabetes. Expert guidance and support will be given by trained WW coaches.

COVID 19 UPDATE: All face to face workshops from March 16th have been cancelled and instead the offer is now diabetes prevention virtual workshops using Zoom video conferencing. Virtual Workshops via Zoom have with numerous sessions available throughout the day, evening and weekend. The digital offer is still available.
There is an intention to support people at risk of diabetes and to reduce the need to attend General Practice to introduce self -referral for who have completed the ‘know your risk’ tool on diabetes UK website https://riskscore.diabetes.org.uk/start and who meet the criteria.
This should be launched the week commencing 6 July. It will be in place up to 31 March 21 and the current referral route from PC remains the gold standard.
