Education for Type 2 Diabetes

 


Diabetes and You


Diabetes and You is the locally developed Structured Education offer for T2DM run by Kernow Health.

It is a free course for adults diagnosed with type 2 diabetes, delivered by trained educators. The course is delivered over 3 x 90-minute sessions (1 session each week). It covers general messaging for anyone with type 2 diabetes giving them the opportunity to better self-manage their diabetes. The priority for delivery is for those newly diagnosed in the past 12 months, however, we welcome people living with T2DM who would benefit from the education to better manage their diabetes.  

Eligibility criteria for Diabetes and You.

•             18 years or above •             People newly diagnosed within 12 months •             People living with T2DM who would benefit from the education to better manage their diabetes

Exclusion:

•           Pregnancy  •           Anyone aged over 80 without prior consideration for the risks and benefits of  a programme •           Active eating disorder


There are face-to-face courses delivered with GP practices as well as virtual courses delivered over Microsoft teams.

You can refer patients to a course or they can self refer using the following link.

Healthy Living

Healthy Living is the first nationally commissioned digital structured education programme. It is an evidence-based, self-directed, web-based programme, and employs behaviour change models and nudge techniques to support users to take action. It provides users with a variety of tools and information to help them better manage their type 2 diabetes, live a healthy life and achieve good wellbeing.

Practices can refer participants to the programme by providing the registration page: https://www.healthyliving.nhs.uk/ where the individual can create an account.



Diabetes UK Learning Zone

The Diabetes UK Learning Zone gives advice on what to eat and tips for managing diabetes day-to-day. There are videos, quizzes and interactive tools tailored to the individual. All courses are completely free.

Diabetes UK also have a new and exciting partnership with Sport England called ‘Live Well Move More’ who are supporting people to become more active. There is a free 10 week online course, exercise hub and physical activity helpline. Email livewellmovemore@diabetes.org.uk  for more information.
