The Royal Cornwall Hospital will be changing the Two Week Wait Suspected Cancer Forms to meet the new requirements of the Suspected cancer: recognition and referral June 2015 NICE guidance
The new forms will be the only versions accepted from the 11th July 2016.
To improve the governance arrangements we will also be ceasing to accept fax referrals from the 11th July 2016. All practises have access to, and most already use, E-Referral system and this will be the only method for referrals after the 11th July 2016 when the fax machine will be switched off.

It is important to have the correct demographics and contact details of your patients and for them to be aware that they have been referred to secondary care to rule out cancer. We aim to offer patients an appointment within 8 days and provide some choice of appointments within the 14 day standard but this can be challenging if there are difficulties in contacting the patient or if they are unaware of the urgency or not available within the 14 days to attend.
