A pathway for new presentations of skin eruptions in adults and children that enables all RCHT-facing practices to gain a rapid opinion on diagnosis and management. It is a useful alternative to a face-to-face clinic referral for selected patients.

It involves sending a digital photo with clinical history through to the RCHT dermatologists using the standard NHS e-Referral system. We will respond within 72 hours.

You can take a picture with a digital camera or even your smartphone. The ‘Quick Reference Guide’ below explains how to do this.

How to do telederm in a nutshell:

1.       Gain consent

2.       Take picture

3.       Dictate letter

4.       Secretary uploads via e-Referral system

In your letter please include the following information:

  • Brief description
  • Other areas involved (mucosae, scalp, nails etc)
  • Duration
  • Precipitating/alleviating factors
  • Symptoms (i.e. pruritus, discomfort)
  • Change over time
  • Results of relevant investigations
  • Treatment used so far and response.
  • Any pre existing skin conditions?
  • Relevant past medical history
